When requesting portal access for your child from our Pediatrics office, the child must be a current patient and the person requesting the invite must be Parent or Legal Guardian on file.
For parent and child, have the name, date of birth, and your email address.

Provide the operator with your first & last name, date of birth, and your email address.
Note: You will receive an invite via email.
