Vaginal birth after C section, or VBAC, is an option for many women

After a woman has a cesarean, or C section, she wonders if she will be able to give birth vaginally in the future. For many women, a VBAC, or vaginal birth after cesarean, is an option that our ObGyns are happy to discuss.

Who is a good candidate for a VBAC?

Each pregnancy and each woman is unique, so the decision to attempt a VBAC must take many factors into consideration. A woman is a good candidate if she meets the following criteria.

She has had one, low, transverse uterine incision. If a woman has had two or more C sections, the odds for success diminish, but she can usually still attempt a VBAC.
She is carrying a normal-size baby who is progressing well during the pregnancy.
She is not currently experiencing the problem that caused her last C section.
She has already had a successful VBAC with a previous pregnancy.

In some cases, a woman may not be a good candidate for a VBAC. For example, if she is carrying a very large baby and/or is past her due date, or if she had a vertical uterine incision during her previous C section, she will likely have another C section.

VBAC benefits and risks

This option allows a woman to avoid major surgery and lowers the risk for infection for the mother. It also allows faster recovery after giving birth.

On the other hand, a VBAC carries certain risks.

If the VBAC is not successful and a repeat C section is necessary, it causes more complications than that of an elective C section.
The risk for uterine rupture is very, very low, but if it does occur, it is a serious medical situation.

The American Congress of Obstetricians and Gynecologists recommends that most women be offered a trial of labor after cesarean, or TOLAC, if they want to attempt a VBAC. A trial of labor simply means that a woman begins her labor with the intention of having a vaginal birth. She and the baby are closely monitored during labor, with the team aware that circumstances that occur during labor may necessitate a repeat C section.

If a woman chooses to try a VBAC with a trial of labor, she needs to discuss this decision with our Grapevine and Fort Worth ObGyns before going into labor so plans can be made to ensure that necessary staff is available.
